Where is Tasha speaking?

 

Periodically, throughout the year, I speak at conferences or Webinars on topics concerning elder care marketing and the Internet. I don't generally get paid for this. I simply enjoy teaching. It's part of our mission to help families get connected with the services they need by helping providers make better use of the Web.

As with all our sessions, there is no programming knowledge required. We simply talk strategy. If you have any questions about any of these workshops, by all means feel free to contact me.

Collaborating (safely) over the Internet

With penalties for frequent readmission, an emphasis on medication management, and the specter of bundled payments on the horizon, CMS' priorities are clear: we need to collaborate to improve transitions, making the patient/family the center of care. There are powerful opportunities for hospice in this area. This session will showcase free and low cost tools available on the Internet to help you promote continuity when a patient is at home and even upon transfer to other settings. From medication management tools to quick status updates and secure multi-provider/patient communication, you add value and quality to your service by keeping everyone in the loop with the convenience of 24/7 web access. We will also discuss HIPAA and compliance issues. No programming experience needed.

By the end of this session, attendees will be able to:

  • Identify at least three current Medicare initiatives that provide financial incentives to collaborate and improve communication and patient support across the care continuum
  • Name the four key factors identified for successful care transitions
  • Explain the advantages and disadvantages of online tools
  • Describe at least 3 Internet tools that can be used securely by patients/families and providers across multiple settings
  • List at least 3 internal procedures that must be in place for online tools to succeed
